Registration and Request to Contact Biological Sibling Who Was Placed For Adoption

If you are a sibling requesting a search for your biological sibling who was placed for adoption, the following requirements apply:

You must be 21 years of age and the sibling for whom you are searching must be at least 18 years of age.

You must complete the Biological Sibling Consent to Contact form and the Registration and Request to Contact Biological Sibling form. When completing the request form, there is a space for you to list questions you may have for your sibling. Please write these questions in the first person, as if you were speaking directly to your sibling. You must specify which sibling you wish to search for. Also, in addition to these forms, please include a letter (also written in the first person) to your sibling including the following information:

  • A physical description of yourself
  • Your current family situation (married, single, children, etc)
  • Your occupation
  • Your motivation for searching
  • What do you hope to gain from searching and making contact?
  • What are your expectations?
  • Provide some information about your relationship with your parents and other family members. Are they aware that you are conducting a search?
  • Any other information you wish to include about yourself

Please return these forms, once the consent to contact form is notarized, along with a $300 payment via check or money order made payable to FAMILIES FIRST to:
