Non-Identifying Information

If you are an adult (18 years and older) and have been adopted in the state of Georgia, background history of your birth parents and their families may be available from your sealed adoption record.  This usually includes physical descriptions, social and health information, early childhood development data and reasons for the plan of adoption. 

There is a fee of $35.00 to research your file and prepare a summary of non-identifying information.  Complete and mail the Registration and Request for Non-Identifying Information Form along with your payment to:

PLEASE NOTE: This form MUST be notarized.  

Please make check or money order payable to Families First. We also accept Visa or Mastercard payments via the telephone. Once we have received your request and your fee, it takes approximately 2 to 3 weeks to complete the summary.
