Registration Only

The registry is a database system maintained by the Georgia Adoption Reunion Registry for the purpose of registering your choice to have contact or not have contact with your birth family member.

There is no fee to register only.

If you would like to register to NOT have contact with a birth family member, please complete, notarize and return the “Affidavit of Non-Disclosure” form to:

If you would like to register to have contact with a birth family member, please complete, notarize and return the “Adopted Persons Registration and Request Consent to Contact” form to:

Please note that once you have registered for contact, if your birth family member is registered we will contact you.
